Identifying organizational risk factors in space support environments: The use of surveys (2004)
Organizational factors can affect system risk. In the aviation environment, organizational factors have contributed to many aircraft accidents. Some of these organizational factors have been lack of training, time pressure, over-scheduling (and resulting fatigue), policies on resource management (fuel use), and faulty procedures (e.g., shift handovers in maintenance, check list procedures in the cockpit)
